Good Seed provides specialized, compassionate care that improves health by working on the social determinants of health. We do it through intensive care management, specialized housing, community engagement and special projects — and every program lives behind one of six doors.

ASCENT — Re-Entry ICMS
Trauma-informed case management for adults leaving jail, prison or detention: housing, behavioral health, employment, legal navigation and family reunification.
