Enhanced Care Management (ECM)
ECM is a whole-person, interdisciplinary approach to care for members with the most complex medical and social needs. It provides intensive, in-person care management where clients live, work and access services. Every member is assigned a dedicated Lead Care Manager who coordinates all aspects of care — physical health, behavioral health, dental, developmental and social services — building a trusted relationship and addressing both clinical and non-clinical needs to improve health and quality of life. Statewide, ECM aims to improve care coordination, integrate services, address the social determinants of health, and reduce emergency room and inpatient visits.

Eligibility — Medi-Cal members in one or more population of focus
- Homeless or at risk of homelessness
- High utilizers of emergency departments, hospitals or short-term skilled nursing facilities (5+ emergency visits or 3+ unplanned hospital stays in the last six months)
- Serious mental illness or substance use disorder
- A medical or psychiatric condition that creates a risk of institutionalization
- Formerly incarcerated and transitioning back to the community
- Pregnant and postpartum individuals who are Black, American Indian, Alaska Native or Pacific Islander (up to 12 months postpartum)
Housing Transition Navigation
From search to keys. A Housing Navigator actively guides eligible members past the barriers to finding, applying for and securing permanent housing:
- Personalized housing search — direct help identifying suitable, affordable options.
- Barrier identification & resolution — assessing needs and removing obstacles, including expungement and employment referrals.
- Application & coordination support — hands-on help with rental applications; liaison between members, landlords and housing authorities.
- Documentation navigation — obtaining ID, income verification and other critical documents.
- Subsidy linkage — connection to housing subsidies and financial programs.
- Move-in logistics, connection to Housing Deposits and Tenancy services, and case management for six months to a year after move-in.
Eligibility — Medi-Cal members who are homeless or at risk, and have at least one of
- One or more serious chronic physical health conditions
- One or more physical, intellectual or developmental disabilities
- Meets access criteria for Drug Medi-Cal (DMC) or DMC-ODS
- Pregnant, up through 12 months postpartum
Housing Deposits
Removing the financial barrier to move-in. Even after a unit is found, upfront costs stop most approvals from becoming keys. Housing Deposits pays security deposits and utility deposits, and provides essential furnishings or medically necessary household items. It is one-time assistance with a lifetime cap of $5,000 per member.
Community Support 03Housing Tenancy & Sustaining Services
Building a stable foundation. Ongoing education and support so that people keep their housing long-term and settle into their community:
- Tenant education — rights, responsibilities and lease compliance.
- Financial literacy & budgeting — managing household expenses.
- Landlord relations — conflict resolution and mediation.
- Eviction prevention — crisis intervention before an issue becomes a loss of housing.
- Community integration — links to ongoing health and social services.
Day Habilitation
Peer-taught life skills in your home or community — cooking, budgeting, transit, self-advocacy, benefits, landlords — for members who are homeless, at risk, or exited homelessness within the past 24 months. Full details and eligibility.
Community Support 05Home Modifications
Environmental Accessibility Adaptations: ramps, grab bars, stair lifts, wider doorways, accessible showers and other physical changes that make a home safe, so members can live independently and avoid moving to a nursing facility. Transitional RentSix months of rent during a transition.
