Good Seed mission is to provide specialized, compassionate care and services that promote better health outcomes by focusing on the social determinants of health. We deliver on this mission through four key approaches: intensive care management, specialized housing, community engagement, and innovative special projects.
Supportive Housing
Good Seed creates a Supportive Housing Environment for our clients to thrive with comprehensive wrap around services.
Our Street Outreach team works directly with individuals experiencing unsheltered homelessness within their current encampments and local hotspots to serve as their primary contact to get connected to much needed services.
Our Drop-In Centers provide a safe place for transition age youth 7 days a week, a place to shower, do laundry, access basic services, resources, and support in finding housing, and connect with community.
The ICMS program assists high and medium acuity individuals ages 18-80 with a mental or physical disability, to obtain and maintain housing, and reduce their reliance on the emergency room as their primary method of health care, through individualized planning services.
