Housing Support
Every client who works with Good Seed completes a housing needs assessment in partnership with trained staff to identify both immediate and long term housing goals. Using an up to date housing resource directory developed with community partners, clientsyouth are supported in exploring a range of housing options and next steps that align with their needs and circumstances.
 
For clients seeking long term housing, Good Seed provides hands on support to secure market rate and affordable housing options. This includes assistance with applications, documentation, and navigating housing programs. Through close collaboration with partner agencies, additional housing opportunities and wraparound supports are made available, expanding each young person’s pathway to lasting stability and independence

Support Groups and Programs

Good Seed offers a variety of support groups and programs, both on site and through a strong network of community partners, for clientsresidents in our care. These spaces are designed to feel welcoming and non judgmental, where asking for help is encouraged and support is viewed as a strength.
 

Clients are connected to services that address the whole person, including mental health care, primary healthcare, substance use support, education, and social and recreational opportunities. Whether provided directly or through trusted partners, these programs help young people build skills, strengthen resilience, and reduce barriers as they work toward their personal goals.

1. Enhanced Care Management

Enhanced Care Management (ECM) provides comprehensive, whole-person care coordination for members with complex health and social needs. ECM offers intensive, in-person care management where clients live, work, and access services. Clients are assigned a dedicated Lead Care Manager who coordinates all aspects of their care across physical health, behavioral health, dental, developmental, and social services. The program focuses on building trusted relationships and addressing both clinical and non-clinical needs to improve health outcomes and quality of life. Core services include:

  • Outreach and engagement
  • Comprehensive assessment and care management planning
  • Enhanced care coordination across all service systems
  • Health promotion
  • Comprehensive transitional care
  • Member and family supports
  • Coordination of and referral to community and social support services

Eligibility Criteria:

Members must be enrolled in Medi-Cal with Health Net, LA Care, Anthem, or Molina and meet criteria for ONE or more of the following populations of focus:

  • Homeless or at risk of homelessness
  • High utilizers of emergency departments, hospitals, or short-term skilled nursing facilities (5+ emergency visits or 3+ unplanned hospital stays in the last 6 months)
  • Have a serious mental illness or substance use disorder
  • Have a medical or psychiatric condition that causes a risk of institutionalization
  • Formerly incarcerated and transitioning back to the community
  • Pregnant and postpartum individuals who are Black, American Indian, Alaska Native, or Pacific Islander (up to 12 months postpartum)

2. Housing Navigation Services

Housing Navigation Services help unhoused or housing-insecure clients find and secure stable housing. A Housing Navigator works directly with the client to conduct comprehensive housing assessments, develop individualized housing support plans, identify suitable housing options, assist with housing applications, and coordinate the move-in process. Services focus on identifying and addressing barriers to housing, securing funding for move-in costs, and ensuring successful housing placement. Services include:

  • Housing assessment and support plan development
  • Housing search and identification
  • Assistance with housing applications and documentation
  • Coordination of move-in logistics
  • Connection to Housing Deposits and Housing Tenancy and Sustaining Services
  • Referrals to Enhanced Care Management and other Community Supports
  • Case management for 6 months to 1 year after securing housing

Eligibility Criteria:

Members must be enrolled in Medi-Cal with Health Net, LA Care, Anthem, or Molina and meet the following criteria:

  • Homeless or at risk of homelessness

AND have at least ONE of the following:

  • One or more serious chronic physical health conditions
  • One or more physical, intellectual, or developmental disabilities
  • Meets access criteria for Drug Medi-Cal (DMC) or DMC-ODS
  • Pregnant up through 12 months postpartum

3. Day Habilitation

Day Habilitation supports clients in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to live successfully and independently in their community. Services are provided in the client’s residence or community settings and focus on building life skills, developing interpersonal relationships, accessing community resources, and supporting overall independence. The program uses a peer mentoring approach and provides a physical location for service delivery when appropriate. Services include: 

  • Life skills training (cooking, shopping, money management, household management)
  • Assistance with how to use public transportation
  • Relationship building, communication, and self-advocacy skills
  • Assistance using local services and accessing benefits (SSI, General Relief)
  • Support selecting and moving into a home, managing landlord relationships
  • Recruiting and managing personal attendants
  • Caregiver support and housing setup assistance
  • Coordination with health, mental health, and substance use disorder services
  • Referrals to ECM, Community Supports, and housing programs

Eligibility Criteria:

Members must be enrolled in Medi-Cal with Health Net, LA Care, Anthem, or Molina and meet ONE of the following:

  • Homelessness
  • At risk of homelessness, and would benefit from participation in Day Habilitation
  • Housed but exited homelessness within the past 24 months
