Good Seed’s Supportive Housing Program goes beyond meeting basic needs. It is a place where adults aged 18 to 65 who have experienced housing instability can feel safe, regain a sense of control, and begin building the skills and confidence needed for long-term stability. We believe that good healthcare starts with access to housing, and residents are active participants in their own growth. With support from a dedicated care team, residents set personal goals and develop practical life skills that support independence, such as managing daily responsibilities, completing financial aid applications, and preparing for college or employment. Each resident is supported at their own pace, with the aim of making their stay at Good Seed their last experience of homelessness and a strong foundation for permanent housing.

Program Overview

The Good Seed Supportive Housing Programs serve all adults aged 18 to 65 who are experiencing or at risk of homelessness and are living with Serious Emotional Disturbance (SED) or Severe and Persistent Mental Illness (SPMI). Residents live in a shared, supervised housing setting in Los Angeles County while receiving comprehensive, wraparound support designed to promote stability and self-sufficiency. Services include individualized counseling, independent living skills development, educational support, and employment preparation. Through our flexible, “as-long-as-needed” model, residents are supported in building the tools necessary to maintain housing, pursue education or employment, and establish strong community connections. Like all Good Seed programs, the focus is not only on housing, but on helping clients thrive long after they leave our care.

1. Short Term Post Hospitalization Housing

The Short Term Post Hospitalization (STPH) program offers individuals experiencing or at risk of homelessness with medical or behavioral health needs a safe and supportive environment to live for up to six months. While STPH is very similar to recuperative care, it does not offer the medical care traditionally found in recuperative care and uses a more peer support group model.

Members should be capable of living independently immediately after discharge from an institution, such as:
  • A mental health or substance use disorder treatment facility
  • Recuperative care
  • Inpatient hospital 

It also provides ongoing support such as Housing Navigation services, socialization and adaptive skills training, necessary psychiatric care, case management, access to medically-tailored meals, and access to other housing supports.

Eligibility Requirements:

Admission to this program is contingent upon meeting ALL the following eligibility requirements and approval from the applicant’s health plan.

  • Enrolled in Medi-Cal with Health Net, LA Care, Anthem, or Molina
  • Homeless or at risk of homelessness
  • Exiting an institution (MH or SUD treatment facility, recuperative care, or inpatient hospital)
  • Have an ongoing physical or behavioral health need as determined by a health professional that would otherwise require continued institutional care
Meet at least one of the following: 
  • Receiving ECM
  • If the member is not currently receiving ECM, we are also able to enroll the member in ECM services
  • Have 1 or more serious chronic condition
  • Have or need mental health services

2. Transitional Rent

Transitional Rent is a short-term housing support that provides rental assistance to support eligible members as they move towards stable housing. Transitional rent services pay for permanent and interim housing for members who are part of a transitional population, are homeless or at risk of homelessness, and who have a qualifying behavioral health need. It can cover rent, short-term housing support, storage fees, amenity fees, and landlord-paid utilities included in rent for six months. This service is meant to support health, safety, and recovery during a housing transition. 

Eligibility Requirements:

Admission to this program is contingent upon meeting ALL the following eligibility requirements and approval from the applicant’s health plan.

  • Enrolled in Medi-Cal with Health Net, LA Care, Anthem, or Molina
  • Homeless or at risk of homelessness
  • Have a medical, behavioral health, or social need that makes housing support necessary
  • Be transitioning out of institutional care, incarceration, interim housing, recuperative care, or the child welfare system
  • Some plans may require medical documentation, lease agreements, and proof of ability to sustain housing after 6 month (e.g., BHSA funding eligible)

3. Enhanced Emergency TAY Shelter

The Enhanced Emergency TAY Shelter provides a safe, supportive entry point for young adults who are experiencing homelessness and need immediate stability. Designed for transition-age youth between the ages 18–25, the program offers short-term shelter in a nurturing environment where youth can rest, feel safe, and begin planning their next steps.

Support is delivered in partnership with youth, honoring their voice, choices, and lived experiences. With guidance from a trauma-informed team, residents receive individualized support, access to mental health services, and assistance with immediate needs such as housing navigation, life skills, and employment or education planning. The shelter is not just a place to stay, it is a starting point for youth to regain stability, build confidence, and move toward longer-term housing and self-sufficiency.

Like all Good Seed programs, the Enhanced Emergency TAY Shelter focuses on more than survival. Our goal is to ensure that every young person leaves with a clear plan, strong support, and the tools needed to pursue a healthy, stable, and meaningful future.

Eligibility Criteria

To participate in the Enhanced Emergency TAY Shelter, youth must meet ALL the following criteria:

  •     Ages 18–25
  •     Diagnosed with, or in need of, mental health services (SED or SPMI)
  •     Experiencing homelessness or at imminent risk of homelessness
  •     No income to pay for housing
  •     Not a danger to self or others and not gravely disabled
